The Patient Care Coordinator reports directly to the Clinical Director who reports to the VP of Rehab Services who reports to the Administrator/CEO.
Qualifications:
Must have at least a high school diploma or general equivalency degree (GED); Be able to read, write, and comprehend English.
Responsibilities:
Develops and implements systems to ensure organization and timely retrieval of documentation.
Participates with intakes PRN.
Responsible for case management of Medicaid, private insurance, and/or private pay clients, including, but not limited to the following tasks:
Sends and procures MD orders according to timelines Participates in scheduling of patient treatment sessions Communicates with therapists regularly for completion of timely documentation, notification of services issues, etc.
Creates and sends accurate plan of treatments to physician and ensures timely receipt from physician Coordinates with Director/Assistant Director for obtaining verbal orders as it relates to the plan of treatment if applicable Creates Patient Summaries according to timelines Seeks authorizations according to timelines Ensures missed visit reports are sent to the MD monthly Monitors hospitalizations and coordinates with the Director/Assistant Director for obtaining verbal orders if applicable Types, mails, emails, and/or faxes appropriate and precise therapy information and/or changes as needed (i.
e.
notes to physician, home programs, etc.
) Communicates with clients and/or families as needed Maintains current patient records in Raintree Documents tasks and communication records correctly and regularly Participate in compliance issues, including, but not limited to, oversees visit frequency accuracy and researches, procures, and ensures timely inclusion into the electronic record all related documentation requirements Assists staff in locating documentation and dispersing per request Audits, manages, and thins clinical records regularly Participates in organizing case conferences and staff meetings PRN Orients new employees, including, but not limited to, reiterates important documentation deadlines, answers questions, reviews office communication issues Additional duties as needed deemed by the Clinical Director, and/or Administrator.
