Legacy Community Health is a premium, Federally Qualified Health Center (FQHC) that provides comprehensive care to community members regardless of their ability to pay.

Our roots began in 1981 as the Montrose Clinic, with specialization in HIV education, testing, and treatment.

Adult primary care, HIV/AIDS care, pediatrics, OB/GYN and maternity, dental, vision and behavioral health.
We also service students within KIPP and YES Prep schools.

Job DescriptionAs a Health Information Specialist III you will be responsible for ensuring all patient medical charts are filed accordingly and efficiently and maintained in a necessary order.
Essential Functions Perform specialized tasks related to patient identity, record completion and/or data integrity at an enterprise-level that include, but are not limited to; chart merge, chart error correction, monitor and maintain assigned work queues, and/or assist with system maintenance.
oFollows the Legacy guidelines related to the Health Insurance Portability and Accountability Act (HIPAA), designed to prevent or detect unauthorized disclosure of Protected Health Information (PHI).
oMaintains strict confidentiality.
oUses oral and written communication skills to effectively convey ideas in a clear, positive manner that is consistent with the LEGACY Mission.
oMaintains established Legacy policies, procedures, objectives, quality assurance, safety, environmental and infection control.
oImplements job responsibilities in a manner that is consistent with the LEGACY Mission and Code of Ethics and supportive of Legacy's cultural diversity objectives.
oSupports and adheres to LEGACY Service Guarantee.
oPerforms other related work as required.

Attends all required meetings and safety training programs and can describe his/her responsibilities related to general safety, department/service safety, specific job-related hazards.
oOperates assigned equipment and performs all procedures in a safe manner as instructed.
oMaintains work area and equipment in condition required by LCHS standards.
oDemonstrates proper body mechanics in all functions.
oReports safety concerns promptly to appropriate personnel.
Follows LCHS's exposure control plans/bloodborne and airborne pathogens.
oDemonstrates knowledge of techniques, procedures and correct use of protective barrier equipment (Universal Precautions).
oAssures a safe environment by instituting appropriate control measures.
Performs other duties as assigned.
Required Education, Certifications, Licenses, & Training High school graduation or GED.
3 years of related experience or in lieu of experience, a degree from an accredited college or university Required Years of Experience Associate's Degree or higher with 3 years of healthcare experience preferred Previous experience in a clinical/he setting preferred.
Registered Health Information Administrator (RHIA) Certification Registered Health Information Technician (RHIT) Certification, preferred Word processing and computer experience preferred.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
