Registered Nurse (Diagnostic Referral Coordinator)

The DRC RN is responsible for providing the clinical leadership and expertise for the coordination of the medical center's diagnostic and referral program using all avenues of resources available to provide safe, efficient, timely coordination of interfacility quality health care and services. Maintains proficiency in the use of interfacility consult and referral programs and to center service around the patient, towards self-managed care, and efficient and cost-effective use of health care resources across care markets within the VA. Directly interacts with the Veterans and their family, and with employees at various VA and non-VA locations. Responsible in planning, designing, integrating, implementing, modifying, and evaluating the appropriateness and timeliness of services provided in response to individual healthcare needs along the illness and care continuum, and/or the effectiveness of program components improving the quality of patient care. Additional duties include, but are not limited to the following:
Serves as the main point of contact for Veteran and VA employees in coordinating referral and consultations between VA facilities Serves as a leader, utilizing sound clinical judgment and established criteria for guidance in developing, implementing, and maintaining programs in the department, including but not limited to Interfacility Referral and Consult Program, Diagnostic Referral Program, Transfer and Traveling Veteran Program Serves as a consultant to primary care and specialty care services regarding clinical and administrative resources that may be utilized in the diagnostic and referral program Supports and collaborates with leadership, functional teams, QSV staff, and all other staff regarding accreditation issues. Stay abreast of changes related to the requirements and standards, presents timely updates of information and insures leadership and all key individual having responsibility are informed. Demonstrates leadership meeting metric goals, data collection, management and analysis and in the timely submission and us of data and information for compliance and for improvement of outcomes Acts as the facility's customer service representative as he/she communicates with Veterans and families, all providers of care, ancillary and support services and external customers to facilitate efficient patient-centered coordination of care Provides guidance and oversight to assigned services/programs to ensure compliance with VHA Handbooks/Directives Leads in huddles, morning reports, or their respective Interdisciplinary Teams (DTs) Meetings; works collaboratively with other coordinators, and other support service staff. Serves as an active participant for numerous Medical Center Committees, or Collaborative Workgroups most relevant to assigned services/programs or clinical expertise. Work Schedule:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated material and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
